Sliding Fee Schedule

No patient will be declined care because of an inability to pay for services. Patients will be expected to comply with the efforts of the Sunshine Clinic staff to ascertain the existence of any third-party insurance (Medicaid, Medicare, private insurance, SCHIP, etc.) coverage a patient may possess, or otherwise appropriately document said patient’s inability to pay for services. Sunshine Clinic does not want financial constraints to prevent individuals and families from seeking the care that is needed; therefore we have adopted a sliding fee discount schedule. Patients qualifying for a sliding fee discount will be expected to pay an affordable flat fee at the time services are rendered. This payment will cover a single provider visit.  There will be an extra minimal charge for ancillary services such as lab and x-rays.  This payment does not cover pharmaceuticals or dental, there is a separate schedule for those services.

Patients with no third party coverage will be expected to provide appropriate information for the completion of the Sliding Fees Scale Eligibility form.  Eligibility is based on proof of income presented (pay stubs or tax returns) and/or social verification records and the most recent Federal Poverty Guidelines. Eligibility forms must be signed by the patients, reviewed and approved by Fiscal Management.   Determination of eligibility for sliding fee scale discounts will be done on an annual basis.
