STRATEGIC PLAN OVERVIEW

 

Goal 1: Staff and Board development - Improve recognition, recruitment and retention of Board and Staff

  • Identify and eliminate barriers to being a Board Member
  • Develop a plan for utilizing technology to connect for meetings 
  • Team Building Workshops for Board and ED- annually
  • Identify ways to recognize service and accomplishments of staff and Board
  • Develop annual Board Calendar for meeting/training and special activities- assign to Board Development
  • Develop succession Plan

Goal 2- Access- Improve access to quality medical, dental and behavioral health services

  • Work Flow redesign
  • Investigate feasibility of services in Trapper Creek
  •  Investigate transportation needs area wide
  • Improve marketing of services and CHC mission
  • Continue to work towards meaningful use and patient centered medical home- develop plan
  • Develop outcome measures to evaluate the quality of patient care and the effectiveness of the organization

Goal 3 Services: Maintain level of treatment services and expand into preventative services (wellness).

  • Develop wellness program
  •  Investigate expansion of dental services
  • Willow Transition plan
  • Old Willow Building
  • Investigate transition from dispensary to full pharmacy
  • Evaluate expansion of specialty clinics
  • Monitor the current level of all services (medical, dental, billing, fiscal, advocacy, etc.) through outcome measures-data collection

Goal 4- Finance- Ensure adequate financial resources to offer all current and future programs

  • 100% chart completion within 3 days of the visit
  • Develop alternative funding resources
  • Plan to maintain salaries and benefits at 95-100% of other AK-CHCs
  • Hire development director by 2013
  • Work towards a method to eliminate the statement information frustration

Goal 5- Facilities- Offer high quality facilities and grounds to meet the needs of services offered

  • Finish new Willow Clinic
  • Identify resources to complete the furnishing of the new facility
  • Develop and implement a five (5) year maintenance plan for all SCHC facilities (inside and out) in Talkeetna and Willow.
  • Investigate development of a duplex housing
  • Develop maintenance plan for the grounds to include outside lighting and signs
