Dental Providers
Paul Forman, MD - Chief Medical Officer
Dr. Forman's philosophy toward health care is to identify the best pathway to health for each individual based on each individual's values, beliefs, and needs. He believes that, as a physician, it is his job to help patients promote health, prevent disease, educate patients about their conditions, and help them select the best treatment options for their individual needs. He provides office-based procedures including trigger point injections, joint injections, skin lesion surgeries, IUD and Nexplanon insertions, and toenail removals when needed. He enjoys all ages from pediatrics to geriatrics and has experience with palliative care as well.
Dr. Forman is affiliated with the American Academy of Family Physicians, the Alaska Academy of Family Physicians, and the American Board of Family Medicine.
After completing his residency, Dr. Forman moved to Alaska and started his career at the Anchorage Neighborhood Health Center. After several years with ANHC and The Alaska Family Medicine Residency faculty, he moved to Seward for about 9 years before moving to the valley. He was one of the founding physicians for Solstice Family Care in Wasilla which has now transitioned to Sunshine Community Health Center. He is very happy to return to community health as this supports his passion for providing high-quality care to people in need.
Dr. Forman lives with his fiancé and pack of great dogs. He has mushed for over 20 years and now finds himself with an aging dog team that still enjoys a good run through the woods, although shorter and slower than they used to run. In the summer He enjoys paddling, hiking, and pretending to fish. On dark winter nights, he is am often found with a guitar doing some songwriting or otherwise ruining good music.

William Frantz, DMD - Dental Director
Raised in a small town south of Pittsburgh, Pennsylvania, Dr. Frantz attended Washington and Jefferson College, graduating in 1993 with a Bachelors of Science in Biology. At the University of Pittsburgh, he attended the School of Dental Hygiene, receiving his R.D.H in 1996, then continued his education to graduate with his Doctorate of Dental Medicine in 2000. Dr. Frantz's previous experience as a general dental practice owner and associate is a tremendous asset to SCHC in providing dental health education and care for the families in the community and surrounding areas.

Insurance Coverage
We accept all forms of payment, including private insurance and Denali Kid Care, Blue Cross Blue Shield, Delta Dental and United Concordia are preferred insurances but we will file all others as a courtesy.
It is beneficial to provide and review your dental insurance information prior to your visit to avoid any surprises. A payment is expected at the time of service.
Medicaid
The Medicaid Fiscal Year is from July 1st to June 30th.
As long as you remain eligible for Dental Medicaid and your Medicaid is active, you will be eligible for four preapproved, billable Dental visits from July 1st – June 30th.
Many, if not most, Dental services are covered by Medicaid. You can receive more than one Dental service during your visit. If you reach your ‘cap’ (all four visits are used up), some emergency services may still be covered by Medicaid. We will bill for these emergent visits, and Medicaid may approve at their discretion.

We are currently accepting new patients within our service area. Our service area is defined as Willow to Cantwell, including remote areas such as Skwentna and Chase.
