Personalized, Ongoing Support for Growing Families

Family Care Management (FCM) is a specialized nursing service led by Abby Bradley, RN-C, IBCLC, designed to provide hands-on support during pregnancy, postpartum, and early newborn care.
Think of FCM as your consistent guide and advocate throughout your pregnancy journey — someone who helps coordinate care, provide education, monitor important milestones, and ensure nothing falls through the cracks.
What Is Family Care Management?
Family Care Management provides education, monitoring, and care coordination alongside your primary provider. It is especially valuable in rural communities where accessing specialty care may require travel or coordination between multiple providers.
How Is FCM Different from a Provider Visit?
A provider visit (with a physician, PA, or NP) focuses on diagnosing, prescribing, and making medical decisions.
Family Care Management complements that care by focusing on education, ongoing support, monitoring, and coordination. Your provider directs your medical care — your Family Care Manager helps you understand it, stay on track, and feel supported every step of the way.

What Can Family Care Management Provide?
Serving Willow, Trapper Creek, and Talkeetna.
Prenatal Support
· Prenatal visits including fetal heart tone monitoring and RN education
· Prenatal panel and appropriate lab coordination
· Ultrasound orders
· Non-Stress Tests (NSTs) — scheduled and occasional unscheduled
· Limited ultrasound for fetal positioning (as appropriate)
· Blood pressure monitoring
· Glucose tolerance testing
· Rhogam injections
· IV hydration when indicated
Education & Birth Preparation
· Childbirth education
· Birth preparation planning
· One-on-one RN education
· Care coordination to ensure appointments and testing are scheduled and completed
Lactation & Postpartum Support
· Lactation consultation (IBCLC certified)
· Postpartum home visits for lactation support
· Postpartum follow-up care
· Newborn check-ins
Care Coordination
· Case management support
· Assistance with records sharing
· Facilitating communication between Sunshine CHC and delivering providers
· Services similar to Nurse Family Partnership (not currently available in our service area)
Getting Started
If referred from outside Sunshine CHC and not yet established with a provider, patients must first establish care with a Sunshine CHC provider. The Family Care Manager will then coordinate next steps.
